Intensive Treatment Unit, UChicago Medicine AdventHealth GlenOaks Hospital, 701 Winthrop Ave, Glendale Heights, IL The role you'll contribute:
Coordinates the overall interdisciplinary plan of care for behavioral health patient from admission to discharge.
The value you'll bring to the team:
Creates plan for care across the continuum, integrating patient/family preferences and values.
Monitors patient care through assessments, evaluations and/or patient records.
Advocates for resources and removal of barriers.
Maintains ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.
Acts as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care.
The expertise and experiences you'll need to succeed:
EDUCATION AND EXPERIENCE REQUIRED:
Master's degree required At least one year of experience required LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
One or more of the following:
Clinical Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
Social Worker specializing in Clinical Practice credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
Social Worker credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
