Behavioral Health Intensive Treatment Unit, UChicago Medicine AdventHealth GlenOaks Hospital, 701 Winthrop Ave, Glendale Heights, IL The role you'll contribute:
Coordinates the overall interdisciplinary plan of care for patient from admission to discharge.
The value you'll bring to the team:
o Creates plan for care across the continuum, integrating patient/family preferences and values.
o Monitors patient care through assessments, evaluations and/or patient records.
o Advocates for resources and removal of barriers.
o Maintains ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.
o Acts as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care.
QualificationsThe expertise and experiences you'll need to succeed:
EDUCATION AND EXPERIENCE REQUIRED:
o Master's degree required o At least one year of behavioral health experience required LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
One or more of the following:
o Clinical Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
o Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
o Social Worker specializing in Clinical Practice credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
o Social Worker credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
